HELP! ACHILLES TENDONITIS?

I had achilles tendonitis about a year ago, and my docter put me in a boot. I play soccer and im running a lot and its starting to hurt agian in the sam part of my same foot. What should i do? Should i get a hard cast to completly mobilize it or just stick with the boot again? Or anything else? Surgery? PLEASE HELP.
